To send us a form duly filed or any other document, we recommand that you send it by mail to our mailing address : P.O. Box 696, Drummondville, Quebec J2B 6W9.
You can also send it by fax at 819-474-1990 or through your financial security advisor. If you can no longer find their contact details, contact us and we will be happy to provide them.

Claim request following a death
How to make a claim request following a death?
- Contact your financial security advisor; or
- Contact our customer service at 1-800-567-0988;
- For insurance contracts issued for less than 10 years or for insurance amounts of 200 000$ or more, please complete the Claimants Statement Death Claim (for life insurance issued less than 10 years ago or for a value of over $200,000). Complete form and send it to UV Insurance office with the death certificate issued by the funeral Home;
- For insurance contract issued for more than 10 years or for insurance amounts of less than 200 000$, please complete the Claimants Statement Death Claim (for life insurance issued more than 10 years ago and valued at less than $200,000) .Complete form and send it to UV Insurance office with the death certificate issued by the funeral Home;
- Complete the Death certificate (Attending Physician) form if one of the following conditions applies to you:
- Contract of $24,999 or less issued during the last 5 years;
- Contract of $25,000 or more issued during the last 9 years;
- Contract of $200,000 or more;
- Contract reinstated during the last 3 years, regardless of the amount.

What will be the next steps?
If some documents are missing to proceed with your claim, we will send you a letter indicating the additional documents required for analysis.
Should I send you the originals?
In general, they are not mandatory. However, they might be required in some circumstances. If you do send the original documents, they will be sent back to you once the claim’s analysis is completed.
When will I receive the insurance payment?
When all required documents will be received, UV Insurance commits to proceed with the claim and issue the cheque as soon as possible.
Claim request following a disability
How to make a claim request following a disability?
- Contact your financial security advisor; or
- Contact our customer service at 1-800-567-0988; or
- Visit the Personal insurance section of the UV Insurance website. Under this tab, select “Claims & Forms” and reach the Disability section. Click on “Claim for Claim for Disability insurance Benefits – Attending Physician’s Statement to access a 4-page document. Complete the “Claimant statement” and “Authorization to obtain and release personal information to a third party” forms and send it to UV Insurance office.
- The “Attending physician’s statement” form will have to be provided to your medical specialist and be sent to us once completed. Make sure your physician indicates an accurate diagnosis and how your health statement restricts your capacity to work. Your physician must also list all courses of treatment you may be following (surgery, medication, physiotherapy, etc.).

What will be the next steps?

When will I receive the reimbursement or follow-up?
When all required medical documents will be received, we will answer you as soon as possible.
Claim request following an accidental fracture
How to make a claim request following an accidental fracture?
- Contact your financial security advisor; or
- Contact our customer service at 1-800-567-0988; or
- Visit the Personal insurance section of the UV Insurance website. Under this tab, select “Claims & Forms” and reach the Fracture section. Click on “Claim Form – Accidental Fracture” to access a 3-page document. Complete the Claim Request – Accidental Fracture to obtain and release personal information to a third party” forms and send it to UV Insurance office with the written report of the radiologist describing the fracture.
- The “Attending physician’s statement” form will have to be provided to the physician seen after the accident and be sent to us once completed.

How to make a claim request following an accident?
- Contact your financial security advisor; or
- Contact our customer service at 1-800-567-0988; or
- Visit the Personal insurance section of the UV Insurance website. Under this tab, select “Claims & Forms” and reach the Accident section. Click on “Claim Form – Accident Insurance” to access a 2-page document. Complete the Claim Form – Accident Insurance to obtain and release personal information to a third party” forms and send it to UV Insurance office.
- Please submit the Claim Form, required documents, original invoices and other supporting evidence within 90 days following the date of accident.

Claim request following the loss of a member, loss of sight or paralysis caused by an accident
How to make a claim request following the loss of a member, loss of sight or paralysis caused by an accident?
- Contact your financial security advisor; or
- Contact our customer service at 1-800-567-0988; or
- Visit the Personal insurance section of the UV Insurance website. Under this tab, select “Claims & Forms” and reach the Accident section. Click on “Claimant’s Statement – Accidental Dismemberment, Accidental Loss of Sight or Paralysis Resulting from an Accident” to access a 4-page document. Complete the Claimant’s Statement – Accidental Dismemberment, Accidental Loss of Sight or Paralysis Resulting from an Accident to obtain and release personal information to a third party” forms and send it to UV Insurance office.
- The “Attending physician’s statement” form will have to be completed by your medical specialist and be sent with the complete medical file fulfilled following the accident.

How to make a claim request following a job loss?

- Visit the Personal insurance section of the UV Insurance website. Under this tab, select “Claims & Forms” and reach the Loss of employment section. Complete the Claimant’s Statement in Case of Loss of Employment in Case of Loss of Employment” and send it to UV Insurance office with your last employment record.

Claim request following a critical illness
How to make a claim request following a critical illness?
- Contact your financial security advisor; or
- Contact our customer service at 1-800-567-0988; or
- Visit the Personal insurance section of the UV Insurance website. Under this tab, select “Claims & Forms” and reach the Critical illness section. Click on “Claimant’s Statement – Critical Illness” to access a 3-page document. Complete the Confidential Medical Certificate – Critical Illness to obtain and release personal information to a third party” forms and send it to UV Insurance office. Then, click on “Confidential Medical Certificate – Critical Illness” to access a 2-page document. The “Confidential medical certificate” form will have to be provided to your medical specialist and be sent to us with your complete medical file.
